Job Description: Care coordinators provide support to individuals
experiencing severe Mental illness (SMI) who are receiving services
from the Assertive Community Treatment (ACT) Teams. As an ACT team
member, the care coordinator works with community resources who
provide services including shelter, housing, detoxification,
mainstream benefits, employment, and transportation. Assists
individuals in daily living activities and establishing and
maintaining linkages that provide education, support, and
independence in the community. Provide case management services for
participants, including assisting participants in obtaining
housing, coordinating with criminal justice agencies, and assisting
clients with activities of daily living such as medication
management and independent living skills. Facilitate recovery
planning for participants On-call rotation: Respond to crisis calls
for assessment and pertinent intervention. Report program
activities and development, crises, and any other pertinent
information to appropriate chain of command. Be available for 24-7
crisis intervention Provides services within the requirements for
the ACT team Participates as an active member of all team meetings.
Proactively involve and communicate with collateral supports,
including family, significant others, probation officers, judges,
caseworkers, and others Document all incidents immediately and
report as required. Complete accurate and thorough client reports,
such as court reports, in required timeframes. Other duties as
assigned. Requirements: Requirements Licensure / Education / Prior
Experience Required: Bachelor's degree in psychology, social work
or related field preferred. Two years or more of related employment
as a care coordinator or interfacing with the community resources
that support reentry into the community preferred. (Housing,
shelter, employment etc.) is preferred Lived experience preferred.
Fluency in Spanish is preferred Trauma Informed Care experience is
preferred. Must be able to drive agency vehicles Employment offers
are contingent upon successful completion of required
pre-employment screenings , which may include background checks,
fingerprinting, applicable sex offender registry screenings for
Adult Residential roles, and other position-related verifications.
Roles requiring driving are subject to a Motor Vehicle Record (MVR)
review and valid Utah driver’s license. All employees of Odyssey

Demands of the Job: Ability to lift up to 15 lbs., drive (or
adequate alternate transportation), sit for prolonged periods, and
